No physician-patient relationship
No physician-patient relationship is created by visiting this website, reading its content, using a scheduler, or submitting a records request. A physician-patient relationship may be established only through an appropriate clinical encounter.
Certification and state decisions
Booking a visit does not guarantee physician certification or state approval. The physician decides whether certification is clinically appropriate. AZDHS controls application requirements, review, approval, and card issuance. Program rules, fees, and forms may change.
Appointments, payment, and fees
Appointment availability, payment-card requirements, payment holds, cancellation rules, and charges are presented during scheduling or in the preparation instructions. Review the current Pricing page and scheduler information before booking. If the physician does not certify you, the doctor-visit charge is not collected and the related authorization hold is released. State application fees apply only when an application is submitted and are separate from the physician visit fee.
Sensitive information
Do not send medical records, identification, payment-card information, or account credentials through ordinary email or a general website message. Use the approved scheduling, records, and document workflows. Never send MMjCCC your AZDHS password.
Third-party services
Heally, the medical-records workflow, AZDHS resources, YouTube, maps, and other linked services are operated by third parties. Their availability, content, privacy practices, and terms are outside this website's control.
Acceptable use
Do not misuse the website, interfere with its operation, attempt unauthorized access, introduce malicious code, impersonate another person, or use the site in violation of applicable law.
Intellectual property
Unless otherwise stated, the website's text, design, logo, and original materials belong to MMjCCC or are used with permission. You may use the site for personal, noncommercial information. Do not reproduce or distribute its materials without permission.
Availability and accuracy
We work to keep information useful and current, but the website may contain errors or become temporarily unavailable. Confirm current program information with AZDHS and confirm appointment details with the office.
Limitation of liability
To the fullest extent permitted by law, MMjCCC is not liable for indirect, incidental, special, or consequential loss arising from use of this website or reliance on its general information. Nothing in these terms limits a right or responsibility that cannot legally be limited.
