Certification and state application fees

We list the physician certification fee separately from the fee charged by the State of Arizona. Confirm the current amount and any discount before booking.

Standard certification visit: $150
Extended visit, when appropriate: Add $50
AZDHS application fee: $150
AZDHS application fee with valid SNAP documentation: $75
Practice discount (one, when eligible): $25
Standard-length combined total: $200–$300

A single $25 practice discount may apply for a senior, veteran, SNAP recipient, or patient living on fixed income due to disability. Discounts do not stack. Extended visits add $50. The standard-length combined total ranges from $200 to $300 depending on the approved practice discount and state-fee eligibility.

State fees are set by AZDHS and may change. Practice discounts apply only when the stated eligibility requirements are met.
