The Arizona certification and application process
A physician certification and an AZDHS application are separate steps. This page explains the sequence and who is responsible for each part.
- Step 1
Book the correct visit
Choose your location and time in Heally. In-person and telehealth visits may be available; tell us when booking so we can confirm which is right for you.
- Step 2
Prepare the required records and identification
The office sends preparation instructions after booking. The physician reviews your medical history and may request supporting records. Use only the approved secure workflow for medical records and identification.
- Step 3
Complete the physician evaluation
The physician reviews the required information and decides whether certification is clinically appropriate.
- Step 4
Complete the AZDHS application
When certified and you authorize submission, MMjCCC submits the state application without an additional application-submission service fee, collects the exact state fee, and pays AZDHS. Patients keep control of their own account credentials.
- Step 5
Watch for state updates
MMjCCC usually submits a complete application within one or two business days after the visit. AZDHS controls application review and approval. Do not promise an approval or delivery date.
